GOALS: Describe as specifically as possible your goals for the following as they relate to ILMA’s mission statement:

Describe in detail your project design and anticipated outcome. If conducting a research project, include a thesis statement
Provide a detailed budget itemizing project expenses. The budget may be inserted below or attached to the application.
Briefly describe the importance of award to project completion.
The applicant must provide (2) two letters of recommendation. The purpose of these letters is to identify reasons the applicant should be considered for a scholarship/grant award. The letters should include a discussion of both the applicant’s qualifications and the value of the proposed project. Acceptable references include: College Advisor, Professor, Supervisor, or Employer. Letters may be included with the application or sent separately to the address on the application

I understand that if I receive financial assistance I must be registered in an accredited college/university as a full time student and maintain a minimum 3.0 GPA each semester. I give my permission for the release of personal information on this form for the purpose of awarding the scholarship. I have attached or sent a copy of my transcript of course records. In addition, I am aware that the deadline for this application is December 31. Late applications will not be considered
